STR DENT HOSPITALS HEALTH SERVICES JOINT STOCK COMPANYFOR PATIENTS CLARIFICATION TEXT PURSUANT TO THE PERSONAL DATA PROTECTION LAW No. 6698
A. Identity of the Data Controller
The “Personal Data Protection Law” No. 6698 (hereinafter referred to as the “Law”) was published and entered into force on 07.04.2016. Within the scope of the said law STR DENT HOSPITALS HEALTH SERVICES JOINT STOCK COMPANY(Hereinafter referred to as "COMPANY" for short.) It has the title of "Data Controller" and carries out the necessary compliance work to fulfill the obligations required by this title. This information is provided to fulfill the COMPANY's obligation to inform its patients within the scope of the Law.
B. Your processed Personal Data, Personal Data Processing Purposes, Collection Methods and Legal Reasons
Your personal data collected for the purposes mentioned below; Name-Surname, TR ID Number, Invoice, Address, Telephone, E-mail address, Past Diseases, Duration of Past Diseases, Drugs Used in Past Diseases, Drugs Used and Amounts, Diseases in Family History, Drugs and Treatments Used in Follow-up, Examination Data, Test Results.
In accordance with the mandatory provisions of the law, it is essential to keep the information of real persons accurate and up-to-date. Therefore, if there is any change in the patients' information, they are required to convey this information to the Company. The COMPANY cannot be held responsible for incorrect information provided under any circumstances.
Your personal data may be processed by the data controller or the legal/real persons he/she will appoint, in accordance with KVKK, in the following cases, within the framework of the personal data processing conditions and purposes specified in Articles 5 and 6 of the Law;
Your personal data, other than your health data, are collected automatically by our patient registrars when you apply to the institution. Your health data is collected through examinations performed by our physicians and questions asked to you, and is processed within the scope of the personal data processing conditions and purposes specified in Articles 5 and 6 of the KVKK.
Your personal data is kept in the security of the data controller and his/her delegated persons.
Your personal data collected by the COMPANY and detailed below are processed for the following purposes and legal reasons in the table.

C. To Whom and For What Purposes the Processed Personal Data Can Be Transferred
Your processed personal data is not shared.
D. Storage Period of Personal Data
Your personal data will be stored for 20 years from the end of your treatment for the above purposes. After the expiration of the period, your personal data will be deleted, destroyed and/or anonymized by the DOCTOR or upon your request, by methods within the scope of the Personal Data Protection Law and relevant regulations. You can always withdraw your consent for the processing of personal data other than data that must be processed by law. If you declare that you do not have permission to process personal data when contacted by phone, your data will be destroyed.
E. Rights of the Data Owner
